Man crushed at CentrePort
A 47-year-old worker was killed at CentrePort Wellington when crushed between a forklift and cargo early on a Sunday morning in January.
A port employee for nine years and reportedly a popular man, father-of-three Mark Samoa could not be revived by emergency services.
CentrePort closed operations for the rest of the day while staff received counselling and an investigation into the death was launched by the port company, Wellington Police and the Ministry of Business, Innovation and Employment.
